Failure analysis is the process of collecting and analyzing data to determine the cause of a failure, often with the goal of determining corrective actions or liability. Failure analysis can save money, lives, and resources if done correctly and acted upon. A failure analysis engineer often plays a vital role in the analysis of failures, whether a component or product fails in service or if failure occurs in manufacturing or during production processing. For E1 students:
Case study 1: Bhopal Gas Tragedy
Description: It was a gas leak incident at Union Carbide India Limited (UCIL) pesticide plant in Bhopal. The highly toxic Methyl isocyanate (MIC) gas made its way into and around the small towns near the plant. Analysis this failure and find the reasons behind this incident and explain the solution in your own way.
Case study 2: Decompression of B-737 of Aloha Airlines by Separation of Upper Fuselage
Description: A sudden decompression occurred in the cabin just behind the cockpit. The maximum hoop stress is exerted on the fuselage and a part of the cabin structure measuring several meters in length was torn off.
For E2, E3, E4 students:
Case study 1: Failure analysis of dissimilar weld in heat exchanger
Description: The failure of dissimilar weld in a heat exchanger has been conducted. Within hours of being placed in service, the circumferential weld joining the carbon steel shell to the duplex stainless steel tube sheet experienced partial cracking as H2S was being introduced into the exchanger.
Case study 2: Investigation of a Columbus, Ohio train derailment caused by fractured rail
Description: A train derailed 17 cars within the city limits of Columbus. Three of cars that derailed were carrying over 86,000 gallons of denatured ethanol in the tank cars ignited, fuelling a large fire. You have to explain the on screen response and failure analysis in detail.
Case study 3: Failure analysis of cracking in aircraft bolts
Description: The failure analysis of the failed clamp bolt from a helicopter engine in the Republic of Korean Air force (RoKAF). The clamp bolt was fractured due to the interaction of tensile residual stress and corrosive environment. Analysis the type of failure and resolve it.
Case study 4: Failure analysis of a leaked oil pipeline
Description: An oil pipeline embedded in an underground trench had failed. There was a perforation at the leak point of the pipeline and some pits collectively located at the exterior surface of failed pipeline. Identify the reason and solution for failure of the leakage of pipeline.
